Patient autonomy is recognised within mental healthcare, although the capacity to participate in one’s own treatment planning is often reduced during a psychotic crisis. The patient may not be sufficiently competent to give consent or express preferences at the time treatment decisions are made. Nine participants were interviewed shortly after a crisis. We discussed participation in the treatment planning and recovery process with particular emphasis on interactions with professionals and understanding treatment. The participants recognised the need for drugs and mental (...) healthcare but emphasised the need for better cooperation and communication. To facilitate the development of patient autonomy, we recommend an increased emphasis on providing information and participating in a dialogue about drug treatment options. This could counteract many of the negative experiences reported. The use of debriefing during hospitalisation and following coercion can be a practical tool for clarifying patient preferences and mutual understanding. (shrink)

In this article, we report findings from a qualitative study that explored how the relatives of intensive care unit patients experienced the nurses’ role and relationship with them in the end-of-life decision-making processes. In all, 27 relatives of 21 deceased patients were interviewed about their experiences in this challenging ethical issue. The findings reveal that despite bedside experiences of care, compassion and comfort, the nurses were perceived as vague and evasive in their communication, and the relatives missed a long-term perspective (...) in the dialogue. Few experienced that nurses participated in meetings with doctors and relatives. The ethical consequences imply increased loneliness and uncertainty, and the experience that the relatives themselves have the responsibility of obtaining information and understanding their role in the decision-making process. The relatives therefore felt that the nurses could have been more involved in the process. (shrink)

An ideal in mental health care is user participation. This implies inclusion and facilitation by clinicians to enable users to participate in decisions about themselves and in the design of suitable treatment. However, much of the work of clinicians consists of handovers and other meetings where patients are not present. It is therefore interesting to study how the patient perspective is handled in such meetings and whether it forms a basis for user participation. We conducted fieldwork in three different inpatient (...) wards in Norwegian District Psychiatric Centres. We used an interactional perspective in our analysis, where speech acts, framing and footing were key concepts. The findings show that the talk in the handovers and meetings contained five main themes and that there was a clear correlation between what was said and how it was said, and whether clinicians related to the content in a decisive, person‐centred or indecisive manner. We discuss potential participation statuses for patients and their limited opportunity to influence the talk and possible decisions about themselves. Our conclusion is that handover meetings primarily function as an aid in organising clinicians' work and could ultimately be seen as counteracting user participation. (shrink)

In this article, we report the findings from a qualitative study that explored how relatives of terminally ill, alert and competent intensive care patients perceived their involvement in the end-of-life decision-making process. Eleven family members of six deceased patients were interviewed. Our findings reveal that relatives narrate about a strong intertwinement with the patient. They experienced the patients’ personal individuality as a fragile achievement. Therefore, they viewed their presence as crucial with their primary role to support and protect the patient, (...) thereby safeguarding his values and interests. However, their inclusion in decision making varied from active participation in the decision-making process to acceptance of the physicians’ decision or just receiving information. We conclude that models of informed shared decision making should be utilised and optimised in intensive care, where nurses and physicians work with both the patient and his or her family and regard the family as partners in the process. Background: Visual technologies are central to youth culture and are often the preferred communication means of adolescents. Although these tools can be beneficial in fostering relations, adolescents’ use of visual technologies and social media also raises ethical concerns. Aims: We explored how school public health nurses identify and resolve the ethical challenges involved in the use of visual technologies in health dialogues with adolescents. Research design: This is a qualitative study utilizing data from focus group discussions. Participants and research context: (...) We conducted focus group discussions using two semi-structured discussion guides with seven groups of public health nurses (n = 40) working in Norwegian school health services. The data were collected during January and October 2016. Discussions were audio recorded, transcribed, and coded into themes and subthemes using systematic text condensation. Ethical considerations: The leader of the public health nursing service who agreed to provide access for the study and the Norwegian Center for Research Data that reviewed and approved the study. All participants gave informed consent. Findings: In adolescents’ use of visual materials with public health nurses, ethical concerns were raised regarding suicide ideations, socially unacceptable content, violation of privacy, and presentations of possible child neglect. The nurses utilized their professional knowledge and experience when identifying and navigating these ethical dilemmas; they resolved ethical uncertainties through peer discussion and collaboration with fellow nurses and other professionals. Discussion: We discussed the findings in light of Annemarie Mol’s interpretation of the ethics of care. Mol expands the notion of ethical care to include the action of technologies. Conclusion: Although the increasing use of visual technologies offered benefits, school nurses faced ethical challenges in health dialogues with adolescents. To address and navigate these ethical issues, they relied on their experience and caring practices based on their professional ethics. Uncertainties were resolved through peer dialogue and guidance. Dette er belyst gjennom intervjuer hvor divergerende meninger gjenspeiler at vi har svært begrenset kunnskap om både kort- og langtidseffekt av screening for kreft. Den politiske viljen til å innføre screeningprogram har utvilsomt vært sterkere enn viljen til å (...) fremskaffe vitenskapelig bevis for eventuell nytteeffekt gjennom randomiserte studier. Randomiserte studier på screening er svært dyre og har ofte et langt tidsperspektiv før resultater kan foreligge og danne et relevant kunnskapsgrunnlag for en nasjonal screeningstrategi. Resultatene kan også være kulturavhengige. Det understreker behovet for flere randomiserte studier. Det må også poengteres at ethvert land som har råd til å vurdere innføring av et screeningprogram, har også råd til å finansiere randomiserte studier i forkant. En omfattende randomisert studie på fleksibel sigmoidoskopiscreening for tarmkreft har vist at deltakelse i screeningprogram kan føre til et redusert egenansvar for sunn livsstil og dermed økt risiko for livsstilssykdommer. Her ligger det altså en pedagogisk utfordring som kan variere fra land til land og mellom forskjellige screeningmetoder. (shrink)

This is the first comprehensive translation of the expanded French version of E.F.F. Chladni's Traité d'Acoustique, using Chladni's 1802 Die Akustik for reference and clarification. Chladni's experiments and observations with sound and vibrations profoundly influenced the development of the field of Acoustics. The famous Chladni diagrams along with other observations are contained in Die Akustik, published in German in 1802 and Traité d'Acoustique, a greatly expanded version, published in French in 1809. The present translation was undertaken by Robert T. Beyer, (...) PhD (1920-2008), noted acoustician, Professor of Physics at Brown University, and Gold Medal recipient of the Acoustical Society of America. The data produced by the scientific community impacts on academia, clinicians, and the general public; therefore, the scientific community and other regulatory bodies have been focussing on ethical codes of conduct. Despite the measures taken by several research councils, unethical research, publishing and/or reviewing behaviours still take place. This exploratory study considers some of the current unethical practices and the reasons behind them and explores the ways to discourage these within research and other professional disciplinary bodies. These interviews/discussions with PhD (...) students, technicians, and academics/principal investigators were conducted mostly in European higher education institutions including UK, Italy, Ireland, Portugal, Czech Republic and Netherlands.Through collegiate discussions, sharing experiences and by examining previously published/reported information, authors have identified several less reported behaviours. Some of these practices are mainly influenced either by the undue institutional expectations of research esteem or by changes in the journal review process. These malpractices can be divided in two categories relating to methodological malpractices including data management, and those that contravene publishing ethics. The former is mostly related to “committed bias”, by which the author selectively uses the data to suit their own hypothesis, methodological malpractice relates to selection of out-dated protocols that are not suited to the intended work. Although these are usually unintentional, incidences of intentional manipulations have been reported to authors of this study. For example, carrying out investigations without positive controls; but including these from a previous study. Other methodological malpractices include unfair repetitions to gain statistical significance, or retrospective ethical approvals. In contrast, the publication related malpractices such as authorship malpractices, ethical clearance irregularities have also been reported. The findings also suggest a globalised approach with clear punitive measures for offenders is needed to tackle this problem. (shrink)

Both American and European investigators have long searched for factors that contribute to musical proficiency. The present article considers several interpretations of musical talent that were advanced by persons who were themselves skilled musicians. Especial emphasis is afforded to the approach of Raleigh M. Drake, an American, who obtained his PhD in Europe, but opposed the most widely utilized early 20th-century American conception of musical talent. Drake also interacted with several early and eminent American psychologists in considering the underlying issue (...) of ‘special talents’. The scholarly contributions of Drake are assessed both in terms of their relationship to some early 20th-century conceptions of musical talent and as a prospective influence on other investigations of that ability. In modern academic career systems there are a large number of entry positions, much smaller numbers of intermediate positions, and still fewer full professorships. We examine how this system has developed in Germany, the country where the modern academic system was introduced, tracing the historical development of academic positions since the early 19th century. We show both a differentiation and professionalization. At first, professorships and private lecturer positions were the only formal positions, but later, lower formal academic positions emerged. Over (...) the whole period, the share of higher academic positions steadily decreased. This differentiation process was closely connected to professionalization: remuneration through salaries was extended from professors to almost everyone working in the German academic system. We propose that the process of differentiation and professionalization was induced by the expansion and democratization of higher education. Finally, our study shows that the opportunities for PhDs to obtain salaried doctoral positions have increased since the 1950s. On the other hand, opportunities for PhDs to become a professor or obtain another tenured job have decreased since the 1980s due to a slowdown of higher education expansion. (shrink)

The still astonishing expansion of the American university since World War II has transformed the nation's intellectual and cultural life in myriad ways. Most intellectual historians familiar with this period would agree, I suppose, that among the conspicuous changes is the sheer increase in the size and diversity of intellectual and cultural activity taking place on campuses across the country. After all, we know that colleges and universities that employ us also provide full- and part-time academic appointments to novelists, poets, (...) playwrights, filmmakers, choreographers, composers, classical and jazz musicians, painters, photographers, and sculptors, even though most of them probably began their careers with little or no desire to join us in the halls of academe. This now widespread employment practice has decentralized the nation's literary and artistic talent. It also has made for a manifold increase in degree-granting programs in writing and the creative arts. One example will suffice here. When World War II ended, there were a small handful of university-based creative-writing programs. Over the course of the next thirty years, the number increased to fifty-two. By 1985, there were some 150 graduate degree programs offering an MA, MFA, or PhD. As of 2004, there were more than 350 creative-writing programs in the United States, all staffed by practicing writers and poets, many of whom now also hold advanced degrees in creative writing. I first left the university to pursue a career in metallurgical research and nuclear technology. But I soon found myself drawn back to the uni versity to 'round out' an overly specialized education. It was along this path that I was 'waylaid' into philosophy by (...) teachers like H. L. Van Breda and Bernard Boelen. The philosophy department at Duquesne University was then (1958-1962) a veritable "little Louvain," and the Belgian-Dutch connection exposed me to (among other visiting scholars) Jean Ladriere and Joe Kockelmans, who planted the seeds which eventually led me to the hybrid discipline of a hermeneutics of natural science, and prompted me soon after graduation to make the first of numerous extended visits to Belgium and Germany. The endeavor to learn French and German led me to the task of translating the phenomenological literature bearing especially on natural science and on Heidegger. The talk in the sixties was of a "continental divide" in philosophy between Europe and the Anglo-American world. But in designing my courses in the philosophy of science, I naturally gravitated to the works of Hanson, Kuhn, Polanyi and Toulmin without at first fully realizing why I felt such a strong kinship with them, beyond their common anti positivism. Carl Henry devotes a few chapters directly (and a few indirectly) in volume 6 of his God, Revelation, and Authority [GRA] to the problem of evil [POE]. The author examines Henry’s contribution as a theologian, noting that GRA is a work of theology, not philosophy proper. However, Henry had a PhD in Philosophy (Boston, 1949), and one finds present several presuppositions and control beliefs that are philosophically motivated. Observation of the text reveals several of these. Chief here is Henry’s working (...) assumption that to understand and explain the nature of evil, one must first understand and explain the nature, origin and etiology of good. This point and its implications are developed at length in this article. Unsurprising is Henry’s contribution exhibiting an awareness of methods and theodical approaches traditionally used by philosophers of religion such as Rowe, Plantinga, and Hick. Surprising is the fact that Henry does not clearly take a side on the nature of human free will. What he does say seems to underdetermine his exact position. Finally, the importance of Kant vis a vis Henry’s theodicy and entire theological program is emphasized as well. (shrink)

This paper examines the contribution of the PhD dissertation of the American cytologist Michael F. Guyer (1874-1959) to the early establishment (in 1902-1903) of the parallel relationship between cytological chromosome behaviour in meiosis and Mendel's laws. Guyer's suggestions were among the first, which attempted to relate the variation observed in the offspring in hybridisation studies by a coherent cytological chromosome mechanism to meiosis before the rediscovery of Mendel's principles. This suggested for the first time that the chromosome mechanism involved a (...) conjugation of maternal and paternal chromosomes during the synapsis followed by a segregation of parental chromosomes in the final germ cells and a random union of the final germ cells in the fertilization. It shows that this early suggestion was similar to William Austin Cannon's later chromosome proposal attempting to explain Mendel's principles and had some influence on Walter Sutton's cytological suggestion explaining correctly the behaviour of Mendel's particle by 1903. Viola Cordova was the first Native American woman to receive a PhD in philosophy. Even as she became an expert on canonical works of traditional Western philosophy, she devoted herself to defining a Native American philosophy. Although she passed away before she could complete her life’s work, some of her colleagues have organized her pioneering contributions into this provocative book. In three parts, Cordova sets out a complete Native American philosophy. First she explains her own understanding of the nature of (...) reality itself—the origins of the world, the relation of matter and spirit, the nature of time, and the roles of culture and language in understanding all of these. She then turns to our role as residents of the Earth, arguing that we become human as we deepen our relation to our people and to our places, and as we understand the responsibilities that grow from those relationships. In the final section, she calls for a new reverence in a world where there is no distinction between the sacred and the mundane. Cordova clearly contrasts Native American beliefs with the traditions of the Enlightenment and Christianized Europeans. By doing so, she leads her readers into a deeper understanding of both traditions and encourages us to question any view that claims a singular truth. From these essays—which are lucid, insightful, frequently funny, and occasionally angry—we receive a powerful new vision of how we can live with respect, reciprocity, and joy. (shrink)

In this essay I argue that hermeneutics, the theory and practice of interpretation, promotes therapeutic interaction between mental health professionals, patients and their family.Why does this patient present in such a way at this particular (...) point in time? The distressing symptoms are there. The changes in behavior are obvious. Impairments of functions are hurting. A mental illness clearly runs in the family. There were early losses of loved ones, physical abuse at home, bullying by peers and self-medication with drugs and alcohol. Three in four adults with a mental illness first experienced that before they turned 24. The brain goes through major reorganization during adolescence and early adulthood. If synaptic pruning went wrong, psychoses emerged. When stresses activated the hypothalamic–pituitary–adrenal axis the elevated cortisol level impacted the size and function of the hippocampus and amygdala in the genetically vulnerable; anxiety, depression, bipolar, and personality disorders developed. The patient needs a voice/language to describe all these experiences and events and the psychiatrist needs a discourse to make sense of them.Eisenberg laments the swings between "brainlessness" and "mindlessness" in psychiatry. Engel argues for a "biopsychosocial" discourse. The major challenge for psychiatry is how to organize clinical information of different nature (categories, dimensions, narratives; i.e., objective vs. subjective, personal vs. interpersonal) into a coherent and comprehensible account for us all. Mental illnesses are not only neuropsychiatric spectrum disorders but also dysfunctions of social relationship and environmental/ecological adaptation. Psychiatry as a clinical neuroscience per se lacks the semantics to address the rich interactions between the physical, psychological, social, cultural, and spiritual in the illness experience of our patients. Explanation in the diagnostic formulation (objective scientific discourse on cause and effect) and understanding in the patient narrative (subjective ordinary everyday discourse on significance and meaning) are irreducible to each other (Wong, 2018). They instead interact with one another to achieve an integrative formulation, not through merging different discourses into a single assertion/monologue but via the dialectic/dialogue of "explaining more in order to understand better," as Ricoeur puts it, and between "the whole and the parts," as Schleiermacher highlights.This "therapeutic hermeneutic circle" of "explanation vs understanding" and "the whole vs the parts" includes symptoms behavior function and life events. It correlates between bio-psychosociocultural-spiritual dimensions/perspectives and does not reduce them to any particular/single [End Page 13] stance. The whole integrates the parts and the parts enrich the whole. It explains but does not explain away. It understands through an articulation of meaning and significance. Keith Donnellan (1931 – ) began his studies at the University of Maryland, and earned his Bachelor’s degree from Cornell University. He stayed on at Cornell, earning a Master’s and a PhD in 1961. He also taught at there for several years before moving to UCLA in 1970, where he is currently Emeritus Professor of Philosophy. Donnellan’s work is mainly in the philosophy of language, with an emphasis on the connections between semantics and pragmatics. His most influential work was his (...) 1966 paper “Reference and Definite Descriptions”. In this paper, he challenges the canonical view, due to Bertrand Russell, about definite descriptions. Russell had argued that the proper semantic treatment of a definite description such as “the present king of France” was quantificational. Thus, a sentence like “the present king of France is bald” should be analyzed as “There exists one and only one entity x that is the present king of France, and x is bald”. Donnellan argues that in natural languages, there are actually two different kinds of uses of definite descriptions. Russell’s analysis picks out the “attributive” use of definite descriptions. When we use a definite description (“the F”) this way, we mean to make statements about the unique entity x that is F. However, Donnellan notes that we also sometimes use definite descriptions “referentially” to pick out a given entity and say something about it. To see this, imagine you are at a party where virtually everyone is drinking beer. However, you and your friend are observing a man in a corner of the room holding a martini glass. Unbeknownst to you, the man’s glass is filled with water. You turn to your friend and ask, “who is the man drinking a martini?” Suppose further that your friend knows that the man in question is Fred and that Fred’s glass is filled with water. According to the Russellian attributive analysis, such a question would amount to asking for the identity of the one and only one man drinking a martini. But the presupposition that there is a man drinking a martini is false, and so there should be no answer to the question.. (shrink)

In particolare viene messo in evidenza che l’intento principale della filosofia fichtiana è quello di edificare un “sistema della libertà”, fondato sul primato del pratico (das Praktische) e dei costitutivi pratici della coscienza umana: l’intuizione intellettuale, l’immaginazione produttiva, lo Streben ed in primis il volere. Viene quindi sottolineato che uno dei vertici della speculazione di Fichte è rappresentato dal concetto di “volere puro” che costituisce il (...) fondamento del conoscere e dell’agire, determinando la natura stessa dell’uomo e dell’assoluto. Nel volume vengono ampiamente utilizzati i nuovi testi messi a disposizione dall’edizione critica delle opere fichtiane curata da Reinhard Lauth: particolare attenzione viene data ai manoscritti di lezioni Dottrina della scienza nova methodo (1796-99) e all’epistolario intercorso tra Fichte e F.H. Jacobi sul rapporto tra la riflessione filosofica e la fede religiosa. Nella parte finale del libro compaiono due Appendici: la prima è dedicata al filosofo bretone Jules Lequier (1814-1862), autore che - ispirandosi a Fichte - elabora una filosofia della libertà tesa a superare una concezione della natura umana deterministica e positivistica; la seconda Appendice è incentrata sull’originale interpretazione del pensiero cartesiano data da Reinhard Lauth (1919-2007): quest’ultimo scorge nel cogito cartesiano il fondamento di una filosofia trascendentale ante litteram che anticipa le posizioni di Kant e di Fichte e che pone le basi per un “pensiero trascendentale della libertà”. -/- AUTORE Tommaso Valentini (Spoleto, 1979) è professore associato di “Filosofia politica” presso l’Università degli Studi “Guglielmo Marconi” di Roma, dove insegna anche “Storia della filosofia moderna”. È docente incaricato di “Ermeneutica filosofica” presso la Pontificia Università Antonianum. Nel 2003 si è laureato in Filosofia sotto la guida del Prof. Armando Rigobello; nel 2008 ha conseguito il titolo di Dottore di ricerca (PhD) in “Etica e antropologia filosofica” presso l’Università degli Studi del Salento. Dal 2006 al 2008, grazie ad una borsa di studio, ha condotto le sue ricerche sul pensiero classico tedesco a Monaco di Baviera presso la Ludwig-Maximilians Universität e la Fichte-Kommission dell’Accademica Bavarese delle Scienze. Dal 2015 è membro del Consiglio nazionale dell’ADIF (Associazione Docenti Italiani di Filosofia). È co-direttore della rivista scientifica on line «Aretè. International Journal of Philosophy, Human & Social Sciences». Ha pubblicato numerosi articoli su Kant, J.G. Fichte, il personalismo e l’ermeneutica filosofica (in particolare Paul Ricoeur). We investigate the polynomial time isomorphic type structure of (the class of tally, polynomial time computable sets). We partition P T into six parts: D −, D^ − , C, S, F, F^, and study their p-isomorphic properties separately. The structures of , , and are obvious, where F, F^, and C are the class of tally finite sets, the class of tally co-finite sets, and the class of tally bi-dense sets respectively. The following results for the structures of and (...) will be proved, where D^ is the class of tally, co-dense, polynomial time computable sets and S is the class of tally, scatted (i.e., neither dense nor co-dense), polynomial time computable sets. 1. is a countable distributive lattice with the greatest element. 2. This paper discusses the important paper by Paul Thagard on the pathway version of mechanistic explanation that is currently used in chemical explanation. The author claims that this method of explanation has a respectable pedigree and can be traced back to the Chemical Revolution in the arguments used by the Lavoisier School in their theoretical duels with Richard Kirwan, the proponent of a revised phlogistonian theory. Kirwan believed that complex chemical reactions could be explained by recourse to affinity tables that (...) catalogued the attraction that various simple bodies possessed towards each other. To explain was in effect to make a delayed prediction, it is not enough just to show how a phenomenon fits into the discernible patterns of the world. Lavoisier, Fourcroy and their colleagues used pathway reasoning, although disguising this fact by suggesting that affinities varied when subjected to n-body situations. (shrink)

Charles R. Kesler, an eminent scholar and prodigious editor, has exerted a profound influence on the study of American politics and the practice of American conservatism. A precocious high-school student, he impressed a visiting William F. Buckley Jr. who, before becoming a life-long friend, wrote him a recommendation letter to Yale. Kesler asked for another--to Harvard, where he completed his undergraduate degree and earned a PhD under the legendary professor Harvey C. Mansfield.
